The Diabetes Clinic serves an adult population suffering from pre-diabetes, gestational diabetes, and type 1 or 2 diabetes.
The clinic team includes a dietitian and a certified registered nurse who are both certified diabetes educators.
In collaboration with a physician and a nurse practitioner, the educators help patients to:
- Acquire and maintain healthy lifestyle habits;
- Understand the illness and effectively control their blood sugar;
- Improve their autonomy;
- Take charge of and control their diabetes;
- Adjust their insulin and learn to manage it themselves.
Diabetes education is free and offered individually or in a group (the person with diabetes can be accompanied by a loved one). Patients do not need a referral from a physician.
Patients can be monitored till their diabetes is stabilized.
