Geriatric Clinics

Our clinics provide specialized services in the assessment, diagnosis and treatment of seniors with memory, mobility and functional autonomy challenges.

Our goal is to improve the health and quality of life of patients.

Our services aim to:

  • Provide access to:
    • A comprehensive geriatric assessment;
    • Rehabilitation services;
  • Foster home support and the independence of seniors;
  • Educate seniors and their informal caregivers on the medical, behavioural, cognitive, physical, social and financial consequences of the disease;
  • Empower informal caregivers in their role and, in partnership with the appropriate community resources, help them enhance their knowledge and skills, while encouraging them to be aware of their limits;
  • Build partnerships between patients, their informal caregivers, health professionals, private residences and long-term care facilities.

Who can receive services?

Anyone 65 years or over who fits the profile of at least one of the following memory, mobility or functional independence problems:

  • Cognitive or functional decline;
  • Behavioural change related to dementia;
  • Decline in ability to drive a vehicle safely;
  • Falls or mobility issues;
  • Polypharmacy that is affecting cognition or mobility;
  • Vulnerability;    
  • Difficulty with mobility or transfers;
  • Difficulty speaking or swallowing;
  • Recent stroke (6 months or less);
  • Risk of family burnout.

Anyone under 65 years old who fits the profile of at least one of the following memory, mobility or functional independence problems:

  • Sudden progressive cognitive decline;
  • Early dementia;
  • Recent stroke with cognitive decline;
  • Specific abnormalities on a radiologic examination of the brain.

Criteria for exclusion from services

 

  • Inpatient
  • Resident of a level 3 or 4 special care home
  • Severe dementia
  • Inappropriate behaviour
  • Frequent fecal incontinence or urinary accidents requiring assistance for hygiene and dressing
  • Patient seen at the clinic recently (less than one year), except if they have had an acute episode since and if they have functional decline
  • Insufficient capacity to participate in the program
  • Under 65 years old
    • Exception(s)
      • Sudden progressive cognitive decline
      • Early dementia
      • Recent stroke with cognitive decline

How to request services

Must be recommended by a physician or nurse practitioner.
