The voice of communities 

A Community Health Needs Assessment (CHNA) is an ongoing, dynamic process aimed at identifying the assets, strengths and needs of communities.

This assessment helps establish health and wellness priorities to improve the population’s health status. It is conducted in collaboration with the community to better understand its needs, and to work together to find solutions.

CHNAs give different population groups the opportunity to have their voices heard in the planning of services and resources in their community.
