All members of the multidisciplinary team participate in the discharge planning process.
By determining the needs of the patient related to discharge from hospital, whether from Emergency or the Medical Unit, it is possible to establish a discharge plan that makes it easier for the patient to return to the community and ensures the continuity of appropriate care.
