Community Health Needs Assessment
A Community Health Needs Assessment (CHNA) is a dynamic, on-going process undertaken to identify the strengths and needs of the community and to enable community-wide establishment of wellness and health priorities that improve the health status of the population.
While the primary goal of the CHNA is to determine a prioritized list of health and wellness needs that can inform decision-makers about resources planning in the community, it is vital that this process enhance community participation and engagement.
